Introduction


Interprofessional collaboration has been essential for the management of cases across various contexts of service delivery. As in the case of healthcare reforms, it is essentially important among health professionals (Ndibu Muntu Keba Kebe et al., 2019). The main purpose of the study is to focus on the importance of cooperation and collaboration in case management. In this study, the role of collaborative practices will be evaluated along with the challenges of collaboration in case management. In this study, the through-care mode of case management will also be addressed. 
 

Collaborative Case Management


Collaborative case management helps in the improvement of the well-being of the people through redirection of access to effective healthcare services (Quilty et al., 2019). Case management remains collaborative in aspects of planning, strategizing, as well as implementing the evaluation of the services. In the entire process, multiple entities are involved which resolves the case as well as closes its state of it. Successful case management depends on the collaboration of the caseworkers with all the stakeholders. Thus, it helps in the improvement of the quality as well as reduction of the cost. 
 

Role Of Collaborative Practice In Case Management

 

Communication


The role of collaborative practices within case management can be reflected in the effectiveness of communication. Innovation within communication technologies acts as a supporting factor in case management (Li et al., 2020). Communication helps in the sharing of information across different levels of case managers who tend to work collaboratively. Communication also assists in serving the same client by the agents of the case management. 
 

Coordinated Service Delivery


In order to collaborate, the actors tend to engage autonomously within a joint creation of norms (Kekez, Howlett & Ramesh, 2018). Thus, the role of collaboration also lies in the aspect of generating coordinated service delivery. Case management involves certain integrated policies, which provide a supportive framework (Duffy et al., 2021). The agents collaborate with each other in the facilitation of access to the services. 
 

Client-Centered Approach


Collaboration within case management helps the case managers to provide a client-centered approach. The client-centered approach reflects the review of the preferences as well as responses towards various care activities (Molony et al., 2018). In client-centered case management, the personal relationship of the case manager with the client reflects the level of collaboration in order to assess the needs of the client. 
 

Importance Of Collaborative Practices In Case Management


Interorganizational as well as interprofessional collaboration remain the important factor in the well-functioning of case management with the given limited resources as well as complicated cases (Karam et al., 2018). Collaboration helps in the joint work of the agents as well as enhances the potential for the creation of seamless client-centered approaches. Collaboration among the case managers improves the coordination and the services in case management. It also helps in well-coordinated plans and the optimal management of the resources. Thus suboptimal collaboration leads to a negative impact on the coordination for the clients. 
 

Examples Of Successful Cooperation And Collaboration

 
Successful cooperation and collaboration in case management can be explored from one of the real-life examples. One relative of mine is a general physician by profession and has worked collaboratively in cancer patient treatment. She has worked collaboratively with the doctors of other departments like a psychologist, oncologists as well as dieticians in treating a single patient. In the case of management, all the doctors have worked with a single aim of having a patient-centric approach leading to the creation of successful collaboration and cooperation in case management. 
 

Theoretical Underpinning

 
The throughcare model reflects throughcare coordination, which helps in overcoming the barriers to proper service delivery like controlling chronic diseases case of healthcare service delivery (Chiyaka et al., 2022). This model is one of the case management models that deal with the case intervention by an agency towards the focus group. The throughcare model takes into account the coordination along with the care management that helps in giving the plan related to personal care (Boustani et al., 2019). 
 
 
Throughcare has been analyzed across various groups including social workers as well as health works. The program under this model helps in targeting a specific group with a common goal of service delivery (Day, Geia & Tamatea, 2019). Effective link, as well as communication prospects, is the primary requirement to be established from the starting point of the throughcare model across a huge range of agencies. Thus, throughcare has been identified as the critical aspect of service delivery. 
 

Challenges To Overcome In Collaborative Case Management   

 
The primary challenges that are included in collaborative case management are misunderstanding, miscommunication as well as low trust. Specific uncertainties among the case managers tend to generate certain misunderstandings and thus leading to a fall in the outcome of collaboration (Russell & Kosny, 2019). The miscommunication among the case managers tends to create more events that are adverse. Poor communication causes the client to receive the wrong service, which negatively affects the outcome. If each of the case managers and agents started to hide information from each other related to a common client, then it affects their trust among themselves. Thus, despite having a huge role in case management, collaboration possesses challenges that affect the whole process of service delivery negatively.
 

Conclusion


Lastly, it can be concluded that the role of collaboration remains essential in case management. The role mainly lies in the aspect of delivering effective communication along with coordinated service delivery as well as a client-centered approach. The importance of collaboration helps in building joint work culture of the agents generating huge potential for client-centered approaches. The throughcare model is one of the models, which highlights the case of intervention toward the target group by a single agency. Certain challenges of collaboration in case management have been identified including misunderstanding, miscommunication, and low trust between the agents. This results in the fall of collaborativeness, which in return inversely affects the service delivery. The clients receive low-quality services because of the associated challenges. These challenges need to be overcome.
